Determination of normal values of thymus tissue using neonatal sonography

ABSTRACT

Aim: The aim of this study was to determine the normal volumetric values of thymus tissue by sonography in healthy newborns.
Material and Methods: A total of 220 healthy newborns were evaluated by sonography in the first week of life. The shape, size, diameter and thymic index values and variations of both lobes of thymus were evaluated in terms of gestational age, height and weight values and standards were calculated for the reference population.
Results: According to the shape on transverse scans; the thymus was quadrilateral in 136 neonates (61%), round in 34 (15%), bilobate in 28 (13%), and crescent shaped in 22 (10%) in transverse scan. On longitudinal scans, the thymus was triangular in 154 infants (70%), teardrop shaped in 35 (16%), oval in 31 (14%). The range of the thymic transverse diameter along with confidence interval was 1.1–3.9 (mean 2.36±0.63), that of the sagittal area was 1.3–4.93 cm² (mean 3.01±0.77), that of the thymic index was 3.1–16.9 cm³ (mean 9.12±2.53), that of the AP (Right) was 0.40–2.40 cm (mean 1.37±0.3), that of the AP (left) was 0.30–3.3 cm (mean 1.69±0.48) and that of the AP (mean) was 0.35–2.83 (mean±1.27).
Conclusion: Knowing the reference values in the neonatal period may be meaningful in evaluating the reactive responses of thymus, which is an immunologic organ in this age population.

ÖZ

Amaç: Bu çalışmanın amacı sağlıklı yenidoğanlarda, timus dokusunun normal volumetrik değerlerini sonografi ile tespit etmektir.
Gereç ve yöntemler: Yaşamın ilk haftasında toplam 220 sağlıklı yenidoğan ultrasonografi ile değerlendirildi. Her iki timus lobunun şekil, boyut, çap ve timik indeks değerleri ve varyasyonları gebelik yaşı, boy ve kilo değerleri açısından değerlendirilmiş ve referans popülasyon için standartlar hesaplanmıştır.
Bulgular: Enine taramalardaki şekle göre; timus 136 yenidoğanda [%61] dörtgen, 34’ünde [%15] yuvarlak, 28’de [%13] bilobat ve çapraz taramada 22’de [%10] hilal şeklindedir. Boyuna taramalarda timüs 154 bebekte [%70] üçgen şeklinde, gözyaşı şeklinde 35 [%16], ovalinde 31 [%14] idi. Timusal transvers çapın güven aralığı ile birlikte aralığı 1,1–3,9 [ortalama 2,36±0,63], sagital bölgeninki 1,3–4,93 cm² [ortalama 3,01 ± 0,77], timik indeksin 3,1-16,9 cm³ [Ortalama 9,12±2,53], AP’nin [Sağ] 0,40–2,40 cm [ortalama 1,37±0,3], AP’nin [sol] 0,30-2,3 cm [ortalama 1,69±0,48] ve AP’nin [ortalama] ], 0.35-2.83 idi [ortalama±1,27].
Sonuçlar: Yenidoğan döneminde referans değerlerin bilinmesi, bu yaş popülasyonunda immünolojik bir organ olan timusun reaktif tepkilerini değerlendirmede anlamlı olabilir.

Incidence and distribution of congenital heart disease in the neonatal intensive care unit: A single center experience

ABSTRACT

Aim: Congenital heart disease is the most common congenital anomaly in newborns. The aim of this study is to evaluate the frequency, distribution and diagnostic clues of the patients diagnosed with congenital heart disease among newborns in the neonatal intensive care unit.
Materials and Methods: 392 infants with congenital heart disease were evaluated retrospectively in our neonatal intensive care unit between January 2014 and September 2018. The reasons of evaluation, gender and cardiovascular system examination, electrocardiogram, echocardiographic examination were recorded.
Results: 392 infants with congenital heart disease were included in the study. Of the patients, 201 were male (51.3%) and 191 were female (48.7%). The rate of congenital heart disease was found to be 11.7% in our center. The most common acyanotic heart disease was ventricular septal defect 31.3%, atrial septal defect 30.1%, and patent ductus arteriosis 21.6%. In cyanotic heart disease, this was tetralogy of fallot 3.8% and transposition of the great arteries 2.5%. The most common reason for evaluation by pediatric cardiologists was cyanosis in cyanotic heart disease, murmur and respiratory distress in acyanotic heart disease.
Conclusion: Congenital heart diseases constitute approximately one third of all congenital anomalies, so it is an important health problem in the neonatal period. In these patients, early cardiological evaluation is very important for diagnosis and early treatment. The incidence of congenital heart disease among infants in the neonatal intensive care unit is higher than in all live births.

ÖZ

Amaç: Doğumsal kalp hastalıkları, yenidoğanlarda görülen en sık doğumsal anomali nedenidir. Bu çalışmada amacımız, hastanemiz yenidoğan yoğun bakım ünitesinde izlenen bebekler arasında doğumsal kalp hastalığı tanısı alan hastaların sıklık, dağılım ve tanısal ipuçları açısından geriye dönük değerlendirilmesidir.
Gereç ve Yöntem: Ocak 2014 – Eylül 2018 tarihleri arasında yenidoğan yoğun bakım ünitemizde yatan 3350 bebek arasında doğumsal kalp hastalığı tanısı alan 392 bebek geriye dönük olarak değerlendirildi. Bu bebeklerin değerlendirilme nedenleri, cinsiyetleri ve kardiyovasküler sistem muayenesi, elektrokardiyogram, ekokardiyografik inceleme ile koyulmuş tanıları kaydedildi.
Bulgular: Doğumsal kalp hastalığı tanısı alan 392 bebek çalışmaya dahil edildi. Hastaların 201’ i erkek (%51,27) , 191’ i kız (%48,72) idi. Merkezimizde doğumsal kalp hastalığı sıklığını %11,7 olarak bulduk. En sık görülen asiyanotik kalp hastalıkları sırası ile ventriküler septal defekt %31,3, atriyal septal defekt %30,1, patent duktus arteriozus %21,6 iken, siyanotik kalp hastalıkları fallot tetralojisi %3,8 ve büyük arter transpozisyonu %2,5 idi. Hastaların çocuk kardiyoloji tarafından en sık değerlendirilme nedeni siyanotik kalp hastalıklarında siyanoz iken, asiyanotik kalp hastalıklarında üfürüm ve solunum sıkıntısı idi.
Sonuç: Doğumsal kalp hastalıkları, tüm doğumsal anomaliler içerisinde yaklaşık üçte bir gibi büyük bir kısmı oluşturmaktadır, bu nedenle yenidoğan döneminde önemli bir sağlık sorunudur. Bu hastalarda erken kardiyolojik değerlendirme tanı ve erken tedavi için çok önemlidir. Yenidoğan yoğun bakım ünitesinde izlenen bebekler arasında doğumsal kalp hastalığı sıklığı tüm canlı doğanlara göre daha yüksektir.

ABSTRACT

Aim: Congenital heart disease is the most common congenital anomaly in newborns. The aim of this study is to evaluate the congenital heart disease in newborns referred to the pediatric cardiology outpatient clinics.
Material and Method: A total of 607 patients admitted to the Pediatric Cardiology Outpatient Clinics between March 2017 and June 2018 during the first 30 days of life were evaluated retrospectively. Electrocardiogram was performed on all newborns. The cardiovascular system examination, electrocardiogram evaluation and echocardiography were performed to all newborns.
Results: 271 (44.6%) of the patients were female. The mean age and weight was 12.25±8.93 days (0-30 days) and 3.57 ± 0.64 kg (1.85-6.0), respectively. The most frequent presenting cause was murmur. 19 of the patients have congenital anomaly. The most frequently detected anomaly was Down syndrome. Echocardiographic evaluation showed no abnormal findings in 168 (27.7%) of the patients. The most common congenital heart disease was atrial septal defect and secondly ventricular septal defect.
Conclusion: Echocardiographic examination was normal in most of the patients. The most frequent presenting cause was murmur. Echocardiographic examination should be performed on patients suspected of having congenital heart disease because the patients may have any different findings and symptoms during neonatal period. Early cardiologic evaluation is very important in the early diagnosis and treatment of congenital heart diseases.

ÖZ

Amaç: Doğumsal kalp hastalığı yenidoğanlarda en sık görülen doğumsal anomalidir. Çalışmamızın amacı çocuk kardiyoloji polikliniğine başvuran yenidoğan dönemindeki olguları doğumsal kalp hastalığı yönünden değerlendirmektir.
Gereç ve Yöntem: Mart 2017- Haziran 2018 tarihleri arasında çocuk kardiyoloji polikliniğine yaşamın ilk 30 gününde başvuran 607 vaka geriye dönük olarak değerlendirildi. Tüm olgulara elektrokardiyogram çekildi. Kardiyovasküler sistem muayenesi, Elektrokardiyogram değerlendirilmesi ve ekokardiyografi yapıldı.
Bulgular: Olguların 271'si (%44,6) kız idi. Ortalama yaşı 12,25±8,93 gün (0-30gün) ve kilosu 3,57±0,64 kg (1,85-6,0) idi. En sık başvuru nedeni üfürüm idi. Hastaların 19'unda ek anomali mevcuttu, en sık saptanan anomali ise Down sendromu idi. Ekokardiyografik değerlendirmede hastaların 168’inde (%27,7) herhangi bir anormal bulguya saptanmadı. En sık tespit edilen doğumsal kalp hastalığı atriyal septal defekt idi, ikinci sıklıkta ise ventriküler septal defekt idi.
Sonuç: Çocuk kardiyoloji polikliniğine en sık başvuru nedeni üfürüm olup olguların çoğunun ekokardiyografisi normal idi. Yenidoğan döneminde doğumsal kalp hastalığı bulguları farklılık gösterebileceği ve hatta hiçbir bulgu vermeyebileceği için şüphe duyulan hastalarda mutlaka ekokardiyografik inceleme yapılmalıdır. Erken kardiyolojik değerlendirme kalp hastalıklarının erken tanı ve tedavisinde çok önemlidir.

Diagnostic value of haematological parameters in neonatal sepsis cases

ABSTRACT

Aim: In this study, we aimed to investigate whether haematological parameters could be used in the early diagnosis of neonatal sepsis in neonates diagnosed with sepsis clinically.
Material and Method: This study was performed retrospectively from the patient records on 138 newborn infants who were diagnosed with neonatal sepsis and admitted to Konya Beyhekim State Hospital Children's Health and Diseases Clinic, Neonatal Intensive Care Unit, between January 2014 and January 2016. Gender, type of delivery (normal vaginal delivery, cesarean section), birth weight, body weight at hospital admission, age at hospital admission, gestational age, duration of hospitalization, accompanying complaints/symptoms, risk factors in the baby and mother were recorded from patient record files. Patients with proven sepsis with positive blood culture results, and who were diagnosed with sepsis clinically without blood culture positivity were included in the study group.
Results: It was determined that 138 (19.43%) newborn infants of the 710 admitted to our unit were diagnosed with neonatal sepsis diagnosis. Of these patients, 82 (59.4%) were male and 56 (40.6%) were female. Male to female ratio was 1.46. While 127 patients were diagnosed with clinical sepsis, 11 (7.97%) of them were haemoculture-proven. The mean birth weights of the patients were 3082,14 ± 423,90 g and 12 (8,69%) patients were below 2500 g. The average body weight of hospitalized patients was 3042,36 ± 422,09 g.
Conclusion: This study once again revealed that haematological parameters are still very valuable in the early diagnosis of newborn infants diagnosed with sepsis.

Amaç: Bu çalışmada klinik olarak sepsis tanısı konulan yenidoğanlarda, hematolojik parametrelerin yenidoğan sepsisinin erken tanısında kullanılıp kullanılamayacağını araştırmayı amaçladık.
Gereç ve Yöntem: Bu çalışma; Ocak 2014-Ocak 2016 tarihleri arasında Konya Beyhekim Devlet Hastanesi Çocuk Sağlığı ve Hastalıkları Kliniği, Yenidoğan Yoğun Bakım Ünitesi’ne yenidoğan sepsisi ön tanısı yatırılmış olan 138 yenidoğan bebek üzerinde hasta kayıtlarından geriye dönük olarak yapıldı. Hasta dosyasından; cinsiyet, doğum şekli (normal vajinal doğum, sezaryen), doğum tartısı, hastaneye yatış tartısı, hastanın kaçıncı gün yatırıldığı, gestasyon yaşı, yattığı gün sayısı, eşlik eden diğer yakınmalar, bebekte ve annede risk etmenler kaydedildi. Kan kültürlerinde üreme olup kanıtlanmış sepsisli hastalar ve kan kültürlerinde üreme olmayan ancak klinik sepsis tanısı konan hastalar çalışma grubuna alındı.
Bulgular: Ünitemize yatırılan 710 yenidoğan bebekten 138’inin (%19,43) neonatal sepsis tanısı ile tedavi aldığı saptandı. Bu hastaların 82’si (%59,4) erkek, 56’sı (%40,6) kız idi. E/K oranı 1,46 idi. 127 hastada klinik sepsis tanısı konulurken hastaların 11’i (%7,97) hemokültür ile kanıtlanmış sepsis idi. Hastaların ortalama doğum ağırlıkları 3082,14±423,90 gr olup, 12 (%8,69) hasta 2500 gramın altında idi. Yatışta vücut ağırlıkları ortalama 3042,36±422,09 gr idi.
Sonuç: Bu çalışma klinik olarak sepsis tanısı konulan yenidoğanlarda, hematolojik parametrelerin yenidoğan sepsisinin erken tanısında halen çok kıymetli olduğunu bir kez daha ortaya koymuştur.
