Demographic characteristics and recombinant tissue plasminogen activator treatment in patients with acute ischemic stroke

ABSTRACT

Aim: Intravenous recombinant tissue plasminogen activator (IV rt-PA) has revolutionized the treatment of acute ischemic stroke (AIS). The use of IV rt-PA in the treatment of AIS is still not at the expected level. Data regarding utilization rate in Turkey is inadequate. In this study, we aimed to present the clinical and demographic characteristics of AIS patients and to share IV rt-PA application data.
Material and Methods: 224 patients diagnosed with AIS between January 2017 and January 2019 were included in the study. Among these, 17 patients received IV rt-PA treatment after informed consent was obtained. Clinical and laboratory findings, risk factors and demographic characteristics of the patients were recorded from our database. Statistical significance level was accepted as p < 0.05.
Results: In this study, the rate of IV rt-PA administration was 7.6% (17 of 224 patients) among all AIS patients. However, in all AIS patients (n = 224), the rate of patients (n = 57) who could application IV rt-PA clinically (National Institutes of Health Stroke Scale [NIHSS] of ≥5 or aphasia or homonymous hemianopsia) was 25.4%. The most common reason for not applying IV rt-PA (72.5%) was that the onset of symptoms was uncertain or the therapeutic time was over 4.5 hours (3 hours in patients over 80 years of age and wide middle cerebral artery infarction).
Conclusion: There are many patients who can benefit from IV rt-PA, which has proven efficacy in the treatment of AIS. In order to increase the number of patients who can benefit from this treatment, multi-faceted studies are needed, including awareness raising activities for the population and training of emergency and ambulance health personnel.

ÖZ

Amaç: İntravenöz rekombinant doku plazminojen aktivatörü (IV rt-PA), akut iskemik inme (Aİİ) tedavisinde çığır açmıştır. Aİİ tedavisinde IV rt-PA kullanımı hala beklenen düzeyde değildir. Türkiye’de ise kullanım oranlarına ilişkin veriler yetersizdir. Bu çalışmada, Aİİ hastalarının klinik ve demografik özelliklerini sunmak ve ek olarak IV rt-PA uygulama verilerini paylaşmak amaçlanmıştır.
Gereç ve Yöntemler: Ocak 2017 ile Ocak 2019 tarihleri arasında hastanemizde Aİİ tanısı konulan 224 hasta çalışmaya alındı. Bunlardan 17’si bilgilendirilmiş onam alındıktan sonra IV rt-PA tedavi uygulanmış hastalar idi. Hastaların klinik ve laboratuvar bulguları, risk faktörleri ve demografik özelliklerine ait bilgilere veri tabanımızdan ulaşılarak kaydedildi. İstatistiksel anlamlılık düzeyi p<0,05 olarak kabul edildi.
Bulgular: Bu çalışmada, tüm Aİİ hastaları içinde IV rt-PA uygulama oranı yaklaşık %7,6 (224 hastanın 17’si) idi. Oysa tüm Aİİ hastalarının içinde (n=224) klinik olarak (Ulusal Sağlık Enstitüsü İnme Ölçeği [National Institutes of Health Stroke Scale, NIHSS] 5 ve üstü veya afazi veya homonim hemianopsi) IV rt-PA uygulanabilecek hastaların (n=57) oranı %25,4’dir. En sık IV rt-PA uygulamama nedeni (%72,5), semptom başlama zamanının belirsiz olması ya da terapötik zamanın 4.5 saati (80 yaş üstü ve geniş orta serebral arter infarktında 3 saat) geçmiş olması idi.
Sonuç: Aİİ tedavisinde etkinliği kanıtlanmış olan IV rt-PA’dan faydalanabilecek çok sayıda hasta vardır. Bu tedaviden faydalanabilecek hastaların sayısını arttırmak için toplumu inme konusunda bilinçlendirme çalışmaları, acil ve ambulans sağlık personelinin eğitimi de dahil olmak üzere çok yönlü çalışmalara ihtiyaç vardır.

Evaluation of fetal nasal bone length during 18⁺⁰-23⁺⁶ gestational weeks

ABSTRACT

Purpose: To establish the nomograms of fetal nasal bone length reported the ethnic differences and relationship between chromosomal abnormalities, between 18⁺⁰-23⁺⁶gestational weeks.
Material and Method: In this study, 2653 fetus between 18⁺⁰-23⁺⁶gestational weeks had normal US findings and had not postnatal anomaly were retrospectively evaluated between January 2009 and September 2014. After detailed fetal anomaly scan, biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), femur length (FL), nasal bone (NB) length and mean gestational weeks (GW) of each fetus were recorded. Regression analyses were performed between NB length and GW, BPD, HC, AC and FL. The reference values for 5-10-25-50-75-90-95. percentiles were established for each gestational week.
Results: In this study, 27 twin pregnancies were detected in the 2626 pregnant women (age range, 17–46 years; mean age, 30.02 ±5.78 years). There were 159, 214, 528, 599, 563, 583 fetuses and the mean NB lengths were 5.5 ± 0.85, 6.3 ± 0.83, 6.6 ± 0.81, 6.9 ± 1, 7.1 ± 0.86, 7.6 ± 0.89 (minimum 4 mm and maximum 10.1 mm) between 18⁺⁰-23⁺⁶gestational weeks, respectively. NB length was increased linearly consistent with GH, BPD, HC, AC and FL during 18⁺⁰-23⁺⁶gestational weeks. In correlation analysis, the fetal NB length were significantly correlated with GW, BPD, HC, AC and FL (p<0.001).
Conclusion: In our study, we established the NB length nomogram in healthy fetuses between 18⁺⁰-23⁺⁶gestational weeks. We think that this reference values can be used for diagnosis of nasal bone hypoplasia in prenatal screening.

ÖZ

Amaç: Etnik farklılıklar gösteren ve kromozomal anomalilerle ilişkisi bildirilen fetal nazal kemik uzunluğunun 18⁺⁰-23⁺⁶. gebelik haftaları arasındaki nomogramlarının oluşturulmasıdır.
Materyal ve Metod: Ocak 2009-Eylül 2014 tarihleri arasında, 18⁺⁰-23⁺⁶gebelik haftalarında, US bulguları normal olan ve doğum sonrası anomali saptanmayan 2653 fetus retrospektif olarak değerlendirildi. Detaylı fetal anomali taraması yapılan her fetusun biparietal çapı (BPD), kafa çevresi (HC), karın çevresi (AC), femur uzunluğu (FL), nazal kemik (NK) uzunluğu ve ortalama gebelik haftası (GH) kaydedildi. NK uzunlukları ile GH, BPD, HC, AC ve FL arasında korelasyon analizi yapıldı. Her gebelik haftası için 5-10-25-50-75-90-95. persentil değerleri hesaplandı.
Bulgular: Çalışmaya dahil edilen 2626 gebenin 27’sinda ikiz gebelik saptandı. Gebelerin yaşları 17-46 yıl (30,02 ± 5,78 yıl) arasındaydı. Çalışmamızda, 18 ile 23. gebelik hafta arasında sırasıyla 159, 214, 528, 599, 563, 583 fetus mevcuttu ve fetal NK uzunluğu ortalama değerleri sırasıyla 5,5 ±0,85; 6,3±0,83; 6,6±0,81; 6,9±1; 7,1±0,86; 7,6±0,89 (minimum 4 mm ve maksimum 10,1 mm) olarak bulundu. NK uzunlukları GH, BPD, HC, AC ve FL artışı ile lineer olarak arttı. Korelasyon analizinde fetal NK uzunluğu GH, BPD, HC, AC ve FL ile anlamlı pozitif korele bulundu (p<0,001).
Sonuç: Çalışmamızda bölgemize ait 18⁺⁰-23⁺⁶. gebelik haftalarında sağlıklı fetuslarda nazal kemik uzunluğu nomogramları oluşturduk. Bu referans değerlerin, prenatal taramada nazal kemik hipoplazisi tanısında kullanılabileceğini düşünmekteyiz.

Determination of S1 screw adjustment parameters using by 3D CT images in healthy subjects

ABSTRACT

Background: The aim of this study was to investigate the anthropometry of the posterior superior iliac spine (PSIS) types in the Turkish population and to determine the safe limits of the length and angle of the sacrum by the posterior lumber approach in lumbosacral instrumentation surgery.
Material and Method: Sacral regions (left and right pedicle lengths and pedicle angles) and PSIS types of healthy subjects were evaluated using 3D images.
Results: A total of 111 patients (58 males and 53 females) were included in the study. The mean age was 46.9 ± 14.8 in males and 44.3 ± 16.4 in females. In men, the right pedicle length was 52.83 mm, the left pedicle length was 53.81 mm, the right pedicle angle was 35.84, the left pedicle angle was 35.13, and the caudal angle was 39.24 degrees. In females, right pedicle length was 48.88 mm, left pedicle length was 49.28 mm, right and left pedicle angle was 35.83, and caudal angle was 39.24 degrees. PSIS type 1 was found to be 58.6%, type 2 32.4% and type 3 9%.
Conclusion: Optimal screw lengths, angles and proper screw orientations can be easily and safely operated in posterior stabilization. In this study, morphometric and anthropometric information related to sacral screw orientation were also determined. These data may help to prevent surgical complications by contributing to the surgeon about preoperative anatomical structure in lumbosacral surgery stabilization procedures.

ÖZ

Amaç: Bu çalışmanın amacı posterior superior iliak omurga (PSIO) tiplerinin antropometrisini Türk toplumunda araştırmak ve lumbosakral enstrümantasyon cerrahisinde posterior lumber yaklaşım ile sakrumun uzunluğunun ve açısının güvenli sınırlarını saptamaktır.
Gereç ve Yöntem: Sağlıklı bireylerde sakrum’un sol-sağ pedikül uzunlukları, pedikül açıları ve PSIS tipleri 3 boyutlu görüntüler kullanılarak değerlendirildi.
Bulgular: Çalışmaya 58’i erkek, 53’ü bayan toplam 111 kişi alındı. Ortalama yaş erkeklerde 46,9 ± 14,8, kadınlarda 44,3 ± 16,4 idi. Erkeklerde sağ pedikül uzunluğu 52,83 mm, sol pedikül uzunluğu 53,81 mm, sağ pedikül açısı 35,84, sol pedikül açısı 35,13, kaudal açı 39,24 derece olarak saptandı. Kadınlarda ise sağ pedikül uzunluğu 48,88 mm, sol pedikül uzunluğu 49,28 mm, sağ ve sol pedikül açısı 35.83, kaudal açı 39,24 derece idi. PSIO tip 1 % 58,6, tip 2 %32,4, tip 3 %9 oranında saptandı.
Sonuç: Posterior stabilizasyonda sakral vidalamada optimal vida uzunlukları, açıları ve uygun vida oryantasyonlarının preop yapılması ile operasyon kolay ve güvenli olabilir. Bu çalışma ile aynı zamanda sakral vida oryantasyonu ile ilgili morfometrik ve antropometrik bilgiler de saptanmıştır. Bu veriler lumbosakral cerrahi stabilizasyon işlemlerinde preop anatomik yapı hakkında cerraha katkı sağlayarak cerrahi komplikasyonları önlemede yardımcı olabilecektir.
