ABSTRACT

Aim: Patients with chronic venous insufficiency (CVE) are susceptible to various disorders due to varicose veins. These complaints affecting the quality of life can occur until leg aches, leg weight feeling, edema, and even chronic ulcers that do not heal. Moreover, varicose veins or surgeries applied to varicose veins can also affect quality of life negatively. We aimed to prospectively assess the impact of quality of life and hospital anxiety scale in patients with CVS in this study.
Material and Method: In this study, 200 randomized patients between the ages of 18-70 who applied to the CVS clinic of Kırıkkale University Medical Faculty Hospital between June 2016 and April 2017 with the complaint of leg pain were evaluated. Patients who were diagnosed as varicose veins during the examination were evaluated objectively by USG with lower extremity venous doppler. Doppler USG and examination were performed and those who had thrombophlebitis, DVT, lymphedema were excluded from the study. Data were assessed by the sociodemographic questionnaire, Life Quality Short Form-36 (SF-36) and Hospital Anxiety and Depression Scale (HAD).
Results: In patients with chronic venous insufficiency, there was statistically significant decrease in physical function, physical role, social function and mental health sub-units in all sub-units except social function, emotional role and mentalkomponent of SF-36 questionnaire <0.05). It was seen that 34% of CVI patients were in the risk group for depression according to HAD-D and 41% according to HAD-A were in the risk group in terms of anxiety. Of the patients who were not diagnosed with CVI, only 28% of the patients with leg pain were depressed and 23% were in the risk group in terms of anxiety.
Conclusion: CVI-associated leg pain is a common problem in the community and can lead to a low quality of life, anxiety and depression tendency. Physical interventions to be applied to the illness alone are not enough. Depression and anxiety also need to be assessed and followed up by the patients and additional measures must be added to the treatment.

ÖZ

Amaç: Kronik venöz yetmezliği (KVY) olan hastalar varise bağlı olarak çeşitli rahatsızlıklar duyarlar. Bacak ağrısı, bacakta ağırlık hissi, ödem, ve hatta iyileşmeyen kronik ülser oluşumuna kadarilerleyebilen bu şikayetler yaşam kalitesini etkiler. Üstelik varis için uygulanan varis çorabı veya ameliyatlar da yaşam kalitesini olumsuz yönde etkileyebilir. Biz bu çalışmada KVY olan hastalarda yaşam kalitesinin ve hastane anksiyete ölçeğinin nasıl etkilendiğini prospektif olarak değerlendirmeyi amaçladık.
Gereç ve Yöntem: Bu çalışmada Haziran 2016 – Nisan 2017 tarihleri arasında Kırıkkale Üniversitesi Tıp Fakültesi Hastanesi KVC polikliniğine bacak ağrısı şikayetiyle başvuran 18-70 yaş arasındaki randomize 200 hasta değerlendirildi. Muayene sırasında varis ön tanısı alan hastalar alt ekstremite venöz doppler USG ile objektif olarak değerlendirildi. Doppler USG ve muayene sonuçlarına göre tromboflebit, DVT, lenfödem tanısı almış olanlar çalışmadan çıkarıldı. Veriler sosyodemografik soru formu, Yaşam Kalitesi Short Form-36 (SF-36) ve Hastane Anksiyete ve Depresyon Ölçeği (HAD) ile değerlendirildi.
Bulgular: Kronik venöz yetmezliği olan hastalarda SF-36 anketinin sosyal fonksiyon, emosyonel rol ve mental komponent hariç tüm alt birimlerinde, kronik bacak ağrısı olduğu halde KVY tanısı almayan hastalara göre fiziksel fonksiyon, fiziksel rol, sosyal fonksiyon ve mental sağlık alt birimlerinde istatistiksel anlamlı düşüklük gözlendi (p<0,05). KVY hastalarının %34’ünün HAD-D’ya göre depresyon yönünden risk grubunda olduğu ve HAD-A’ya göre ise %41’inin anksiyete açısından risk grubunda olduğu görüldü. KVY tanısı olmayıp sadece bacak ağrısı ile gelen hastaların ise %28’inin depresyon, %23’ünün ise anksiyete açısında risk grubunda olduğu izlendi.
Sonuç: Kronik venöz yetmezlikle ilişkili bacak ağrısı toplumda yaygın olarak karşılaşılan bir sorun olup düşük yaşam kalitesi, anksiyete ve depresyon eğilimine yol açabilmektedir. Hastalara uygulanacak fiziksel müdaheleler tek başına yeterli değildir. Depresyon ve anksiyete yönünden de hastaların değerlendirilip takip edilmesi ve bunlara yönelik ek önlemlerin de tedaviye eklenmesi gereklidir.
