The role of abdominal radiography in the diagnosis of acute appendicitis: Is it important?

ABSTRACT

Aim: Most of the patients presenting to the emergency department with an preliminary diagnosis of acute appendicitis (AA) are undergoing standing plain abdominal radiography (SPAR), but SPAR's contribution to the diagnosis of AA is being questioned today. The aim of our study is to evaluate the importance of SPAR in the diagnosis of AA to elucidate this controversial issue.
Material and Methods: We evaluated preoperative SPAR findings and pathology results of patients who were operated with the diagnosis of AA between 2014 and 2017 at General Surgery Department of Ordu University Training and Research Hospital. The relationship between the presence/absence of the SPAR findings and the diagnosis of AA were analyzed.
Results: 140 (77.8%) of 180 patients who were operated for suspicion of AA underwent SPAR. Of the 105 patients with SPAR findings, 90 (85.7%) had AA and 15 (14.3%) were no. No findings were found in the 35 of patients who underwent SPAR, 25 (71.4%) had AA and 10 (28.6%) did not. There was no statistically significant difference in the diagnosis of AA in patients with and without SPAR findings (p = 0.098). Of the 140 patients undergoing SPAR, 115 (82.1%) had AA, and 25 (17.9%) were no. Of the 40 patients without SPAR, 27 (67.5%) had AA and 13 (32.5%) did not. Whether or not SPAR was performed did not affect the diagnosis of AA (p = 0.07).
Conclusion: SPAR rarely helps in diagnosis of AA. SPAR should not be routinely taken to every patient considered AA. If there is another cause of acute abdomen other than AA as a pre-diagnosis in the physician's mind, SPAR may be taken because it is cheap, accessible and easily interpretable.

ÖZ

Amaç: Akut apandisit (AA) ön tanısıyla acile başvuran hastaların çoğuna ayakta düz batın grafisi (ADBG) çekilmektedir; fakat ADBG’nin AA tanısına katkısı günümüzde sorgulanmaktadır. Çalışmamızın amacı tartışmalı olan bu konuyu aydınlatmak için ADBG’nin AA tanısındaki önemini irdelemektir.
Gereç ve Yöntemler: Ordu Üniversitesi Eğitim ve Araştırma Hastanesi Genel Cerrahi Bölümünde 2014 ve 2017 yılları arasında AA ön tanısıyla ameliyat edilen hastaların preoperatif çekilen ADBG bulguları ile patoloji sonuçları kıyaslandı. ADBG bulguları ile AA tanısı arasındaki ilişki irdelendi.
Bulgular: AA şüphesi nedeniyle ameliyat edilen 180 hastanın 140 (%77,8)’ına ADBG çekildi. ADBG de bulguya rastlanan 105 hastanın 90’nın (%85,7) da AA varken 15’ inde (%14,3) yoktu. ADBG’de herhangi bir bulguya rastlanmayan 35 hastanın ise 25’ inde (%71,4) AA varken 10’ unda (%28,6) yoktu. ADBG bulgusu olan ve olmayan hastalar da AA tanısı koyma açısından istatistiksel olarak bir fark yoktu(p=0,098). ADBG çekimi yapılan hastaların 115’ inde (%82,1) AA varken 25’ inde (%17,9) yoktu. ADBG çekimi yapılmayan hastaların ise 27’ sinde (%67,5) AA varken 13’ ünde (%32,5) yoktu. ADBG çekimi yapılıp yapılmaması AA tanısına etki etmemektedir (p=0,07).
Sonuç: ADBG, AA tanısını koymada nadiren yardımcı olur. AA düşünülen her hastaya ADBG çekmektense hekimin aklında ön tanı olarak AA dışında başka bir akut batın nedeni daha varsa ucuz, erişilebilir ve kolay yorumlanabilir olması nedeniyle ADBG çekilebilir.

The efficacy of nonenhanced computed tomography in the diagnosis of acute appendicitis

ABSTRACT

Aim: To compare the effectiveness of contrast-enhanced (CCT) and nonenhanced computed tomography (NECT) in the diagnosis of acute appendicitis.
Material and Method: Patients’ reported to have acute appendicitis on abdominal computed tomography (CT) between January 2017 and February 2018 were identified from the radiology database.The images and clinical characteristics of 149 consecutive identified patients (Male/Female=88/61) were evaluated. The specific CT findings of acute appendicitis were recorded separately for NECT and CCT images by one radiologists without knowledge of the identity and final diagnosis of the patients. The sensitivity, specificity, positive and negative predictive values, and accuracy of CT diagnosis for acute appendicitis were compared to the clinical and surgical findings.
Results: Eighty-one (54.4%) of the 149 patients (Males/Females=48/33,age range:18–74 years; mean age:33.6 years) were acute appendicitis. There was no significant difference in mean age, appendiceal diameter, and presenting appendicolith between males and females in NECT and CCT groups (p ranged from 0.113 to 1). Sensitivity, specificity, positive and negative predictive values and accuracy for the diagnosis of acute appendicitis were 97.9%, 94.6%, 95.9% and 97.2%, 96.5%, respectively in patients with NECT; 96.9%, 95.8%, 96.9%, and 95.8%, %96.4, respectively in patients with CCT.
Conclusion: The diagnostic sensitivity, specificity, and accuracy of NECT are similar to CCT for the diagnosis of acute appendicitis. NECT has several advantages for patients with suspected appendicitis in a busy emergency department setting.

ÖZ

Amaç: Akut apandisit tanısının konulmasında intravenöz kontrast madde kullanılarak ve kullanılmadan elde edilen bilgisayarlı tomografi incelemelerinin etkinliğini karşılaştırmaktır.
Gereç ve Yöntem: Acil servise 1 Ocak 2017 ile 1 Şubat 2018 tarihleri arasında akut nontravmatik karın ağrısı ile başvuran, akut apandisitit ön tanısı ile bilgisayarlı tomografi incelemesi yapılan, 18 yaş üstü hastaların medikal kayıtları ve görüntüleme bulguları retrospektif değerlendirildi. Çalışmaya 149 hasta (Erkek/Kadın=88/61) dahil edildi. Bilgisayarlı tomografi görüntüleri, hastaların daha önceki bilgisayarlı tomografi raporunu ve nihai tanısını bilmeyen bir radyolog tarafından değerlendirildi. Akut apandisit tanısında kontrastsız ve kontrastlı bilgisayarlı tomografinin duyarlılığı, özgüllüğü, pozitif ve negatif tahmini değeri, doğruluğu analiz edildi.
Bulgular: Bilgisayarlı tomografide 81/149 hasta (Erkek/Kadın=48/33, yaşları 18–74 arasında; ortalama yaş 33,6) akut apandisit olarak değerlendirildi. Kontrastsız ve kontrastlı bilgisayarlı tomografi ile akut apandisit tanısı konulan erkekler ile kadınlar arasında, yaş, apendiksin çapı, apendikolit görülmesi bakımından anlamlı farklılık saptanmadı (p>0,05). Cerrahi sonuçlarına ve hastaların klinik takiplere göre kontrastsız ve kontrastlı bilgisayarlı tomografinin akut apandisit tanısı koymada duyarlılığı, özgüllüğü, pozitif ve negatif tahmini değeri, doğruluk oranı sırasıyla; %97,9, %94,6%, %95,9 ve %97,2, %96,5 ; %96,9, %95,8, %96,9 ve %95,8, %96,4 bulundu.
Sonuçlar: Akut apandisitin tanısında kontrastsız bilgisayarlı tomografinin, kontrastlı bilgisayarlı tomografi kadar tanısal doğruluğu ve duyarlılığı bulunmaktadır. Hasta yoğunluğunun fazla olduğu acil servislerde kontrast madde kullanılmadan da akut apandisit tanısı koyulabilir.

A different view of the sonographic classification of the appendix

ABSTRACT

Aim: We intended to detect various appendix localisations with a classification system different from those used in previous literature to facilitate the sonographic detection of the appendix.
Patients and methods: The study was performed on 362 consecutive patients who applied to our department for abdominal or pelvic US examination to our department. The sonographic criterion used to diagnose a normal appendix was visualization of the full extension of a compressible, blind-ending tubular structure with a maximum transverse diameter of 6 mm. Appendices were evaluated by US and localisations were recorded and classified according to the reference line passing through the iliac vessels in the right iliac fossa.
Results: Each appendix was classified as type 1 to 8 according to its location. Type 1 crossed the iliac vessels (85.5%), type 2 was medial to the iliac vessels (2.41%), type 3 was inferior and lateral to the cecum (1.93%), type 4 was in the right paracolic gutter (4.34%), type 5 was completely retrocecal (1.93%), type 6 was in front of the cecum (1.45%), type 7 extended to the umbilicus (0.97%) and type 8 was subhepatic with cecal malposition (1.45%).
Conclusion: The study demonstrated a new classification system (types 1–8) different from those described in previous literature. The most common position of the normal and abnormal appendices in our study was crossing the iliac vessels (type 1). The second most common position was the right paracolic gutter (type 4). Two interesting localisations extended to the umbilicus (type 7) and appeared in the subhepatic space (type 8).

ÖZ

Amaç: : Biz bu çalışmada farklı lokalizasyonlardaki apendiksleri inceleyerek önceki literatürden farklı bir sınıflama yapmayı ve böylece appendiksin sonografik incelemesini kolaylaştırmayı amaçladık.
Materyal ve metod: Çalışma bizim departmanımıza pelvik veya batın ultrasonografisi amacı ile başvuran 362 ardışık hasta üzerinde gerçekleştirildi. Normal sonografik apendiks kriterleri maksimum çapı 6 mm olan, komprese olan, kör sonlanan tübüler yapı olarak belirlendi. Appendiksler ultrasonografi ile incelendi ve kaydedildi, referans çizgisi olarak da sağ iliak fossadaki iliak damarlar alındı.
Bulgular: Apendiksler yerleşim yerine göre 1-8 sınıfa ayrıldı. Tip 1 iliak damarları geçen (85.5%),tip 2 iliak damarların hemen yanında (2.41%), tip 3 çekumun inferior ve lateralinde (1.93%), tip 4 sağ parakolik olukta (4.34%), tip 5 tamamen retroçekal (1.93%), tip 6 çekumun anteriorunda (1.45%), tip 7 umblikusa uzanan (0.97%) and tip 8 çekal malpozisyonla birlikte subhepatik yerleşim (1.45%).
Sonuç: Bu çalışmada literatürde daha önceden tanımlanandan farklı olarak yeni bir sınıflama sistemi (1-8) ortaya konulmuştur. Normal ve anormal apendikslerin en sık görülen yerleşimi iliak damarları geçen tip 1 olarak izlendi. İkinci en sık görülen yerleşim ise sağ parakolik olukta izlenen tip 4 idi. İki farklı ilginç yerleşim ise umblikusa uzanan tip 7 ile subhepatik alana uzanan tip8 idi.
